Vizag Doctors Perform First Hybrid Surgery for Aortic Dissection

Doctors at Apollo Hospitals performed a complex hybrid procedure to treat chronic Type B aortic dissection, the first such surgery in the city.

A 43-year-old man with hypertension had earlier suffered a Type A aortic dissection, a life-threatening tear in the inner wall of the aorta. Doctors treated him with the David procedure, replacing the aortic root while preserving his native aortic valve.

During follow-up visits, the patient reported persistent chest pain and rapidly rising blood pressure. Doctors struggled to control his hypertension with medication. CT scans at three and six months showed chronic Type B aortic dissection extending from the left subclavian artery to the aortic arch.

Cardiologist Dr. Chakradhar Pedada and his team planned a hybrid intervention because of the condition’s complexity.

In the first stage, surgeons performed surgical de-branching of the neck vessels. They placed a graft from the ascending aorta to the innominate artery and the left common carotid artery. Through a neck incision, they connected another graft between the left subclavian artery and the left common carotid artery. This step ensured a continuous blood supply to the brain and upper limbs. The surgery lasted about five to six hours.

In the second stage, doctors shifted the patient to the cath lab for endovascular repair. They inserted a stent graft through the femoral artery and extended it from the ascending to the descending aorta. The stent successfully sealed the dissection.

The patient recovered well, and the doctors discharged him in stable condition within five days.

Speaking to the media on Monday, Dr. Chakradhar Pedada said hybrid procedures combine open surgery and endovascular techniques to treat complex aortic diseases. He said the approach reduces recovery time and complications. He added that this was the first such procedure in the region.

Cardiologists Dr. D.K. Baruah, Dr. N.K. Panigrahi, Dr. Jaideep Trivedi, and Dr. Bhargavi attended the briefing along with chief operating officer Dr. Krishna Prasad.
