A.P. to Launch ‘Sanjeevani’ Digital Health Programme in June
The State government will roll out the Sanjeevani digital healthcare programme across Andhra Pradesh from June, aiming to modernise services at Primary Health Centres (PHCs) and Urban Primary Health Centres through complete digitisation of patient care.
The initiative, being implemented in partnership with the Tata Group through the Tata Digital Nerve Centre (DiNC), was piloted in Chief Minister N. Chandrababu Naidu’s constituency, Kuppam, where it showed encouraging results.
In the next phase, nine PHCs and 66 Urban Health Centres in the district have been identified. District Medical and Health Officer P. Jagadeeshwara Rao said key personnel, including the DPMO, DPO, and medical officers, have been trained.
Under the programme, patient care will be tracked digitally from registration to discharge. Patient Care Coordinators (PCCs) will be deployed at each centre to manage data and assist patients. Mid-Level Health Providers in rural clinics and staff nurses in urban centres will serve as PCCs, while Patient Care Managers will oversee implementation at the constituency level.
Patients will be issued ABHA IDs, enabling doctors to access medical records instantly across facilities. Diagnostic reports, prescriptions, and treatment details will be uploaded to a centralised platform.
If doctors are unavailable, patients can consult specialists virtually through the State-level DiNC centre. The system will also support appointment scheduling, health camp information, and follow-up care.
Officials said the paperless system would improve efficiency and ensure continuity of care. Patient records will be accessible anywhere through ABHA IDs, helping deliver more reliable and timely healthcare services.
